- From the Navigation Bar, click Templates.
- In the Side Navigation, click NPS and Patient Satisfaction Survey. You will see a template editor for your Patient Satisfaction Survey.
- Enter questions in the empty question fields to add more questions to your survey. Edit the low and high rating label, if desired. Empty fields are ignored on the live form.
Note: All questions are based on a 0-10 scale for answers: the low rating label will appear next to the 0 and the high rating label will appear next to the 10.
